Booking an Appointment
Visit our website and navigate to the appointment booking section. From the available options, select the appointment type and your preferred date and time.
Provide your contact information and any relevant details requested during the booking process.
Confirm your appointment booking and await a confirmation notification via email.
Filling Out Questionnaires Before the Appointment
Upon confirmation of your appointment, you will receive access to our secure online portal where you can complete pre-appointment questionnaires.
These questionnaires are designed to gather important information about your medical history, symptoms, and any specific concerns you may have.
Please take time to complete the questionnaires accurately and thoroughly and submit them no later than 12 hours before your scheduled appointment. This will help our clinicians better understand your needs before your appointment.
Appointment with the Clinician
On the day of your appointment, log in to the Semble platform or arrive at our practice at the scheduled time.
Meet with one of our experienced clinicians for a detailed assessment and discussion of your concerns.
During this appointment, the clinician will listen to your symptoms, ask relevant questions, and conduct any necessary assessments to reach a diagnosis.
Together, you and the clinician will explore treatment options and develop a personalized plan tailored to your needs and preferences.
Receipt of Treatment Plan Letter
Following your appointment, the clinician will compile the agreed-upon treatment plan.
You will receive a detailed letter outlining the diagnosis, recommended interventions, and next steps.
This letter serves as a valuable reference and ensures clear communication between you and your clinician regarding your treatment journey.
Receipt of Prescription, if applicable
If medication is deemed necessary as part of your treatment plan, the clinician will provide you with a prescription.
You may choose to have the prescription filled at a pharmacy of your choice or utilize our convenient prescription delivery service.
Follow the prescribed medication regimen as directed, and don’t hesitate to reach out to your clinician with any questions or concerns.
Please note, If the patient is prescribed medication, they will need to book a follow-up appointment to evaluate the response to treatment.
